How to submit your application
- Download and complete the form.
- Return it one of two ways:
- In person: 106 Haley Lane
- By email: cchs@tisd.net (PDF or clear photo scans are fine)
What happens next
- Our team will review your application and reach out with any quick questions.
- We’ll schedule a meet-and-greet with the pet you’re interested in (and any family members or resident pets, if appropriate).
- If it’s a great fit, we’ll go over the final paperwork and next steps so you can welcome your new best friend home.
Helpful tips
- Please make sure all fields are complete and easy to read.
- If you rent, having your landlord’s approval/pet policy handy can speed things up.
- If you have a particular dog or cat in mind, include their name on the form.
